Purpose:
The purpose of this Request for Proposal (RFP) # 58823021 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Department) to select the most qualified Applicants to provide 1st Five Healthy Mental Development Initiative (1st Five) services.
The basics of 1st Five include:

Engaging, training, and providing technical assistance to primary care providers on the 1st Five program and screening and surveillance tools for children birth to age five

Targeting the population of children birth to age five (up to a child’s fifth birthday)

Increasing use of surveillance and standardized developmental screening in primary care

Providing a one-step referral resource for primary care providers

Connecting referred children (and their parents/caregivers) to existing services in their local communities

Keeping primary care providers informed about children’s progress
