Description
The Iowa Department on Aging is seeking a qualified Bidder to implement the evidence-based falls prevention program known as the CAPABLE program in Dallas and Polk counties and surrounding areas in central Iowa.
The objective of this solicitation is to engage the Respondent to help reduce falls and falls risks among older adults and adults with disabilities by utilizing the CAPABLE program.
Iowa counties targeted for inclusion in this project include Polk and Dallas counties.
This approach teams a nurse, an occupational therapist, and a home modification specialist to address the home environment and uses the strengths of the older adults themselves to improve safety and independence.
